Comanche County Memorial Hospital has made access to your medical records and imaging as easy as possible! Patient privacy is a priority when releasing records, therefore we have a variety of ways to access your records that each require identification before release. Whether you have been a patient at CCMH, MMG speciality clinics, or LCHC, you can access your medical records all in one place!

Patients or their legal representatives may request access to medical records and imaging information via the form below. Please note: we require the person requesting the form to upload a photo ID. You can find a list of acceptable documents below.
